MSH CR is the smart companion for Metro South Health Cardiac Rehabilitation patients as they recover from a heart attack or heart procedures such as a bypass, valve replacement, stent, pacemaker, and more. MSH CR can also assist cardiac rehabilitation patients living with conditions such as heart failure, angina, and other forms of cardiovascular disease.
MSH CR will help you can keep track of all the information you'll need for your rehabilitation, including:
- Medication list, schedule, and adherence
- Health measures such step count, blood pressure, weight, heart rate and more
- Data from HealthKit/Apple Health app such as daily step count (with your permission) so you don't have to enter this in manually
- Diary entries for your goals and non-emergency symptoms
- Exercise prescription with adherence
You'll also be able to access education articles and videos to help you and your family better understand your condition/procedure, plus mindfulness exercises that will help you manage stress and anxiety.
MSH CR can help you stay on track during cardiac rehab, whether you’re attending a face-to-face program at an MSH community centre, doing a home-based program with telephone support for your rehab team, or switching between those options as you prefer.

Note: MSH is responsible for providing public health services to the people who reside within its boundaries. Due to high demand, it is not possible to accept referrals from outside this catchment area. Special consideration is made for patients requiring tertiary care or services that are not provided by their local Hospital and Health Service.
